Quick summary
  • One incident log serves Clinical Safety and Privacy & Security, and each incident shows in the areas ticked for it.
  • You can link an incident to a hazard if it’s clinical, or a risk if it’s about data security, from its Incident Details sheet.
  • Severity levels, their meanings, statuses and incident causes are all customisable.
From Privacy & Security, the log opens with Security selected, and you link incidents to risks in your risk register.

What counts as an incident

  • Tick Clinical Incident if it has clinical harm implications.
  • Tick Data Security Incident if it affects the confidentiality, integrity or availability of data.
Tick both if both apply, and the incident appears in each area. You must tick at least one.

What you record

Each incident has a customisable Status and these fields:
  • Severity: hover over a level to see its definition, or customise the levels and their meanings.
  • Root Cause Analysis: write it in the incident, or attach it as evidence.
  • Source: where it was reported, such as Staff Report or Trust Page Report.
  • Cause(s): pick from your incident causes (separate from your hazard log’s), or add one with Create New.
In an incident’s Incident Details sheet, click Link or add hazards under Clinical Details or Link or add risks under Data Security Details, then pick an existing one or create one.
