> ## Documentation Index > Fetch the complete documentation index at: https://docs.assuric.com/llms.txt > Use this file to discover all available pages before exploring further. # Create a hazard > Add a hazard to your hazard log: describe it, add its causes and controls, and score its initial and residual risk. **Quick summary** * Click **New Hazard** and work through 6 stages, from the hazard itself to its residual risk. * **Save & Exit** updates the working log only. To make it official, [approve a version](/clinical-safety/hazard-log-approvals). * You need a written justification from an *Undesirable* residual rating (3 on the default matrix) upwards, or the hazard won't save. Clicking through the stages of an existing hazard can save changes to it. Open a hazard only when you intend to work on it. ## Before you start * Open **Clinical Safety → Hazard Log** and pick your products under *Product*. The table and any new hazard start with them selected. * A hazard can apply to several products. It appears in each product's log, and each log is approved and published separately. ## Add a hazard Click **New Hazard** and fill in the **Hazard** stage. You can add causes once the required fields are set. | Field | What to enter | | - | - | | **Products** (required) | The products the hazard applies to | | **Hazard ID** | Suggested for you, following your [hazard ID pattern](/clinical-safety/hazard-ids-and-statuses#hazard-ids) | | **Tags** (optional) | Labels for filtering, such as Patient Impact or Usability | | **Hazard** (required) | The hazard itself, in a short line | | **Description / Effect** | A short description, or the effect associated with the hazard | | **Harm** (required) | The effect in the care setting, and the potential clinical impact or harm to the patient | The Hazard stage of a new hazard with products, hazard ID, hazard, description and harm completed **Suggest Hazards** and **Suggest Harm** draft from your [product description](/getting-started/adding-a-product). Add each cause in one of the three ways shown below: type a new one, link or copy an existing one, or copy in an AI suggestion. The Causes stage of a hazard with the New Cause for this Hazard field and Add button, the Link or copy existing Causes to this Hazard panel with its product filter, and the Suggest Causes button A [linked cause](/clinical-safety/causes-and-controls#copy-or-link) is one cause in several places, so editing or deleting it changes it everywhere. For each cause, add the controls already in place, in the same three ways as causes. The Existing Controls stage with one cause expanded, the New Existing Control field and Add button, the Link or copy existing Controls to this Cause panel with its filters, and the Suggest Controls button [Attach evidence](/clinical-safety/causes-and-controls#evidence) to each control now, or later from the **Controls** tab. For each control, set: | Field | Options | | - | - | | **Responsibility** | *Manufacturer* (you operate it) or *Deploying Org* (the deploying organisation operates it, so it's a [transferred control](/clinical-safety/exporting-a-hazard-log#what-customers-download)) | | **Control Type** | The kind of control, chosen from a list | | **Control Status** | Yours to define, for example *Awaiting Evidence* or *Done* | The Edit Control form with the Responsibility, Control Type and Control Status fields Rate the hazard with its existing controls in place (**View Causes** lists them). Set *Severity* and *Likelihood* with the sliders, or click a grid cell to set both. *Initial Risk Justification* is optional. The slider descriptions are cut short: **Learn More** gives Assuric's full definitions, and [Risk scales and thresholds](/clinical-safety/risk-scales-and-thresholds) has the matrix. The risk stage with both scales set and the overall rating calculated Under each cause, add controls that bring the risk down further with **New Additional Control**. They take the same fields as existing controls. The Additional Controls stage with the New Additional Control field and Add button, and the Link or copy existing Controls to this Cause panel with its filters Rate what remains with the additional controls in place (**View Causes and Controls** lists them), setting *Severity* and *Likelihood* as before. From an *Undesirable* residual rating ([3 on the default matrix](/clinical-safety/risk-scales-and-thresholds#what-happens-at-3-and-above)), *Residual Risk Justification* becomes required. Confirm whether the risk is as low as reasonably practicable (ALARP) and give a risk-benefit justification, or the hazard won't save. Set *Hazard Status* ([a default or one you've added](/clinical-safety/hazard-ids-and-statuses#statuses)), then click **Save & Exit**. This updates the working log only.
